GVC Fax Modem, 14400 V.42bis/MNP5, Model F1114V/R2. This modem comes with a power adapter and a serial modem cable, 25 pin (DB-25) connector on each end. I believe this modem was marketed towards Windows and is Microsoft compatible, please do your own research. Compatibility may be greater by using a different modem (serial) cable.

Modem is tested to power on, unable to test further as my phone service is not capable of dial-up.
